A successful transition to independent self-care requires patients to embrace the educational materials provided by Skilled Nursing centers. After all, following discharge, a nurse is no longer present to ensure compliance, but the organization still assumes the risk of a readmission and its financial implications. That means patients must engage with instructional materials to fully understand their self-care tasks.

How can health literacy help?

Health literacy is more than determining a patient’s reading level – it means writing or selecting materials for engagement. Clinical accuracy is only the beginning. How you say it determines whether patients will listen or tune out. For this purpose, health literacy must take a new approach to content. Just as you would determine an appropriate reading level, you have a responsibility to deliver materials that appropriately address the patient’s perspective. This session will cover the critical aspects of selecting or writing patient self-care instructions for transitioning to independent self-care.
